Please refer to the "Embedding and Interfacing Manual",
chapter "External Predicates in C and C++" and the corresponding
example file <eclipsedir>/doc/examples/eg_cc_external.cc
This should answer your questions.

You have probably been looking at the documentation for
external/2, which happens to contain C code that
uses an out-of-date version of the C interface.
Sorry about that, we'll update it.
